I'll tell you the procedure we would use to clean our hard wood floors at a bar and club I used to work at. You can't get much dirtier than that. Here is how I have modified it for home use.

You will need:
Murphy's oil Soap
a large bucket
A rag mop, I like the liberman mop with the ringer sleeve
A bunch of old towels.

Thoughly sweep the floor.
Fill bucket with the proper ratio of hot water and soap (read the direction on the bottle)
Wet mop the floor scrubbing the gros spots. Do this in about 6 square foot area, then wipe the floor up with a towel. The towel will absorb all of the dirty water so that it doesn't just dry on the floor.
Change the water in the bucket often.

Tile: Mr. Clean
A deck brush (you can get them at home depot they are a long handled scrub brush.)
A mop
bucket
Bleach 1/4 cup

Mix the proper ratio of water and soap and 1/4 cup of bleach
Dip the deck brush into the bucket scrub the floor in about 4 square foot patch, mopping after scrubbing. Change water as needed.
